Admission Guide at Pantai Hospital Ipoh

Admission

  • For inpatient and day surgery admissions, please approach our reception counter located at the main hospital lobby
  • Admission times: 24 hours daily
  • In case of emergency, please proceed to the Emergency Department
  • Please check in to the hospital at the time advised by your doctor
  • All admissions require a deposit and/or letter of guarantee
  • Payment can be made by cash or any major credit card

Personal Items

  • You may like to bring personal items such as pyjamas and toiletries as the hospital does not provide them
  • Please do not keep large sums of money, jewellery and other valuables during your stay in the hospital
  • The hospital will not be responsible for the loss of any items

Discharge

  • Discharge time is 12 noon
  • Half-day room rate will apply for discharge between 1pm – 5pm
  • Full-day room rate will apply for discharge after 5.00pm
  • Upon your doctor certifying your discharge, it will take up approximately 2 - 4 hours to process your bills and prescriptions
  • When the bill is ready, the staff will inform you and a discharge slip will be issued for you to settle your bill

Follow-up

  • Before you leave the hospital, an outpatient appointment may be made for follow-up treatment
  • Please bring along your appointment card on your next outpatient visit

Birth Registration

All births have to be registered at Jabatan Pendaftaran Negara (National Registration Department). Upon registration, the newborn will be issued a Birth Certificate. Normal registration of birth refers to registration that is made within 14 days of the birth of a child in Malaysia.

Documents Required :

CHILD’S DOCUMENTS (ORIGINALS AND COPIES)

  1. Birth registration form JPN.LM01 that is completed;
  2. Confirmation of birth form from the hospital or certification of home birth from the midwife/doctor (original);
  3. Prenatal card (maternity examination book) (original and copy)

PARENTS’ DOCUMENTS (ORIGINALS AND COPIES)

  • Identity cards or entry permits or passports of the parents and the person reporting the birth (originals and copies);
  • Death Certificate of mother and father of the child (original and copy if applicable)
  • Marriage or divorce certificate (originals and copies)

INFORMANT’S DOCUMENT (ORIGINAL AND COPY)

  • Identity card or entry permit or passport (original and copy)
